TELEHEALTH CONSENT
Consent for Virtual Care Services
Please review the following information regarding telehealth services provided through ValoraCare.
What Is Telehealth
Telehealth allows patients to receive healthcare services remotely using secure audio and video communication technology. This ensures access to medical expertise regardless of physical location within the state of Kansas.
Telehealth Limitations
Telehealth has inherent limitations compared to in-person examinations. Some clinical conditions may require a physical evaluation to ensure a safe and accurate diagnosis. Your provider may recommend in-person follow-up, urgent care, or emergency services if telehealth is determined to be insufficient for your needs.
Technology Risks
Interruptions or technical failures may occur during virtual visits. Internet or video quality can affect the clarity of communication. While ValoraCare implements reasonable safeguards, complete 100% security against all technical breaches cannot be guaranteed.
Privacy & HIPAA
ValoraCare utilizes HIPAA-conscious systems designed specifically to protect patient privacy. All personal health information is handled in accordance with strict privacy practices and legal regulations to maintain confidentiality.
Prescriptions & Treatment Decisions
Prescriptions are issued only when medically appropriate in the clinical judgment of the provider. Providers reserve the right to decline medication or treatment requests. There are strict legal limitations regarding the prescription of controlled substances via telehealth.
Emergency Situations
ValoraCare does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or seek immediate in-person medical attention at the nearest emergency department.
Kansas Patient Requirement
In compliance with state medical regulations, patients must be physically located in the state of Kansas at the time of the telehealth visit.
Consent Statement
By participating in telehealth services, you acknowledge that you understand the risks, benefits, and limitations of virtual care and explicitly consent to receive care via telehealth through ValoraCare.
